Abstract

  • Poster E-Walk oral presentation: Impact of procedural pain on mesenteric tissue oxygen saturation in preterm neonates.ESPNIC, Athens, Greece. June 2023 Purpose: Premature neonates experience up to 14 tissue-damaging procedures (TDPs) per day while hospitalized in the neonatal intensive care unit (NICU) [1]. We previously published that (TDPs) such as tape removal increase plasma markers of hypoxia [2]. However, the impact of TDPs on the oxygenation of specific organ systems was not yet understood. Because the most common TDP performed in the NICU is a heel lance [3], we examined the effect of this TDP on mesenteric tissue oxygenation (mStO2). Methods: Mesenteric oxygen saturation (StO2) was measured in eight premature neonates before and during a clinically required heel lance, and up to three minutes from the end of heel lance, using near infrared spectroscopy (NIRS)[4]. Average percent change and maximum percent desaturation in mStO2 during heel lance was calculated. Neonatal pain was scored at baseline and during procedural heel lance using the Premature Infant Pain Profile-Revised (PIPP-R) [1]. Subjects were provided non- pharmacologic pain support (facilitated tucking or 30% dextrose) to reduce signs of pain. Results: Mean (SD) percent change in mStO2 for neonates tested was - 4.2% (10.1) during and three minutes after heel lance. The average minimum and maximum percent change during that period was 12.1% and -19.5%, respectively. However, individual neonates experienced maximum (SD) percent reduction in mStO2 of -9.0% (14.9) with a minimum and maximum of -13.4% and -28.6%. Gestational age correlated significantly with the mean percent change in mStO2 (Pearson correlation: 0.738, P = 0.037), suggesting that larger reductions in mStO2 during heel lance are associated with lower gestation or greater prematurity. More importantly, a significant negative correlation between procedural pain score and maximum percent reduction in mStO2 is observed (Pearson correlation: -0.901, P = 0.037), indicating that the higher the pain score the larger the mesenteric oxygen desaturation. Conclusion: Neonates experiencing a single heel lance undergo a reduction in mesenteric StO2. The impact of such mStO2reductions on organ functions are not currently known, either singly or following repeated instances. Because preterm neonates are at risk for gastrointestinal disorders, such as necrotizing enterocolitis, appropriately powered studies are needed to investigate this further. (06/2023) (link)
